Every camper is required to complete a "Release and Medical Authorization" form.  Please print, complete, sign, and mail to:

Registration is not complete and you will not be allowed to participate until we have this form. If you are registering online, a form will automatically be emailed to you.

If your Coach or Team Representative is registering your team for the Team Tournament and/or Team Camp, ALL RELEASE AND MEDICAL AUTHORIZATION FORMS MUST BE RECEIVED TOGETHER IN THE SAME ENVELOPE.  Any single Release and Medical Authorization Form received for the Team Tournament and Team Camp will not be processed!
